How to Receive Your GE Appliances Visa Prepaid Card

1. When mailing in your rebate request please ensure that you have the following:
   • Complete ALL information on the rebate form—including model number and serial number for product purchased. Incomplete forms will not be processed. Mark the model purchased on this form.
   • A copy of your original Sales Receipt or Invoice which shows the model number and the name of the store where the appliance was purchased.
   • Include a photograph of the previous cooktop you replaced with the cutout dimensions prior to modification.
   • Include a dated invoice or receipt from a professional installer or contractor stating the cost to modify your existing cabinet on a separate line item from the installation costs, with a photograph of the finished install.

2. Rebate submission must be postmarked no later than July 31, 2023 Late submissions will not be accepted.
